The most recent one I dated: met at a party few years ago (didn't ask her out); met her at an event a while after (didn't ask her out); then met her at another party about a year ago (asked her to be my date to a wedding). Went out for a while and we just recently ended.

While finishing residency, I had this favorite little health food restaurant that I would go to on the weekends. The manager started talking to me about one fo the girls who worked there who was asking about me. I was not planning on staying in town after finishing school, but thought we might have fun for a few months. I learned she was finishing her dual degree and in her last semester, and planning on going back to Dallas. So we started dating, and have beem married for 4 years with a 2 year old son and 1 month old baby girl.

Most recent GF I met at brunch and this was after swearing off dating for a bit (last girl stole my blender!). She was at the table next to me with her friend, and I with my friend. It was the weekend before the NYC marathon and my first comment to her was "Don't shit your self like I did in 2004!"
We ended up exchanging emails and she wrote to me after the marathon. Going on 8 months now, but probably the only non bar girl I have met in NYC. Other than that I always end up meeting women at the bar below my apt... sometimes a terrible idea because they always remember where I live, but always a great way to move on to next steps..."So would you like to have a nightcap upstairs?"

High school, saw her in the hallway and just couldnt keep my eyes off her. Turns out she was a friend of a friend, got her number and found out her address. Went and parked across the street from her house every night and called her. Initially she was freaked out but with perseverance it became the norm and one day I gave up. The next day she calls and says she missed having a stalker, we started dating and just passed 3 years. True story.
